Private patients may refer themselves by telephoning 01227 452529 or ask your dentist for a referral.

NHS patients must be referred by their dentist.

Clicking on one of these links should open a template referral letter for you to fill in on your computer screen, print and send to us by post. Please print a copy for your records before closing the form.

Alternatively print a blank letter and fill it in by hand. The blank letters can also be saved onto your computer for offline use. Depending on how your computer is set up, the previous patient's details may be displayed when a form is re-opened, but you can overwrite this.

Most internet browsers should open these documents automatically using Adobe Reader. If you don't have it Adobe Reader can be downloaded free via the Adobe web site.
